Anyone making strategic decisions for their own practice, such as what role aligner therapy should play in the range of services, benefits from a sober look at the market structure. The following figures come from an industry analysis in a manufacturer publication, as of 03/2026, and a complementary assessment from the same series. The authors themselves point out that the values are based on public industry reports and aggregated estimates and depend on methodology. The point is to recognise structure, not decimal places.

At a glance

  • Around 20 million new orthodontic treatments start worldwide every year (industry estimate, as of 03/2026).
  • The split is roughly 75 to 25: around 15 million bracket cases versus around 5 million aligner cases.
  • Invisalign holds about 13 % of the total orthodontic market, equivalent to around 52 % of the aligner segment.
  • Germany is Europe's largest aligner market; Europe is growing at around 20 % per year.
  • Around 86 % of aligner therapy runs through dental practices, not through direct-to-consumer offerings.

Aligners are growing, but brackets still dominate

Of the roughly 20 million new treatments each year, about three quarters go to fixed appliances and one quarter to aligners (industry estimate, as of 03/2026). For all the attention it receives, aligner therapy is thus in a growth phase, not a dominance phase. Notably, bracket sales are actually still rising in several markets; the overall market is expanding rather than one technology displacing the other. The bracket side is innovating too, for instance with self-ligating systems, a growing ceramic share and customised, 3D-printed brackets with AI-assisted planning. The analysis forecasts that within ten years the aligner share is likely to move towards 35 to 40 %, driven by adult orthodontics, digital workflows and emerging markets.

One market leader and the "missing middle"

Within the aligner segment, the concentration is striking: Invisalign holds about 13 % of the total orthodontic market, i.e. around 52 % of the aligner segment, and operates in more than 100 countries. The second-largest provider, Angel Aligners, accounts for about 2.3 % of the total market, making the market leader more than five times the size of its nearest competitor. Behind them follow providers such as Spark, Smartee, ClearCorrect, SureSmile and Impress at roughly 0.3 to 2 % each, plus a long tail of micro-providers (all shares industry estimates, as of 03/2026). Remarkably, this entire long tail combined roughly equals the market leader's share alone.

This is precisely where the structural anomaly of the market lies. Mature industries such as smartphones or automobiles show a typical pattern: a clear market leader, two to three strong challengers with 20 to 30 % market share, then the long tail. In the aligner market, this middle layer is almost entirely absent. The strategic question of the coming years is who will occupy this "missing middle". The analysis considers consolidation very likely, with ecosystems combining manufacturing, technology and clinical expertise as the likely winners, not necessarily fully integrated corporations.

Geography: regional champions instead of global duels

Below the market leader, the market is strongly regional: Angel Aligners and Smartee are strong primarily in China and the Asia-Pacific region, Impress comes out of the European direct-to-consumer world, other providers out of the US DTC wave. Particularly relevant from a DACH perspective: the European aligner market is growing at around 20 % annually, faster than the global average, and Germany is the continent's largest aligner market (industry estimate, as of 03/2026).

The most important number for the practice: 86 percent

After the ups and downs of the mail-order providers, one metric has remained remarkably stable: around 86 % of aligner therapy still runs through dental practices rather than online direct-to-consumer providers. Adults account for about two thirds of all aligner cases. Taken together, these two facts describe the real market: adult patients choosing clinician-led treatment. The global market volume is estimated at around $5.5 billion for 2025, on course for about $18 billion by 2033, roughly 15 % growth per year (industry estimate from public industry reports, as of 03/2026).

What this means for your own strategy

Three conclusions can be drawn from the structure. First: aligners are not a saturated field; at a 25 % share of procedures and double-digit growth, most of the development still lies ahead of the industry, particularly in the German market. Second: the concentration on one dominant provider does not mean there is only one path; on the contrary, the "missing middle" shows how much room between market leader and long tail remains unoccupied. Third: the 86 percent figure underscores where aligner care takes place: in the practice, under clinical leadership. What role aligner therapy plays in your own range of services and which system is used in each individual case always remains the professional decision of the treating dentist; market figures provide context, not an indication.
